  • POSITION SUMMARY
    • Position Summary:
      • The Care Navigator will connect with patients in the practice. The primary focus of the role will be to coordinate care, manage transitions, quality measures, vaccine management, and support patients with complex needs, often focusing on population health management and improving outcomes within an integrated care team model. Work closely with high-risk patients to support them in coordinating their care, addressing barriers, and helping them become more involved in their health care.
