Clinical Evaluator - Per Diem

Job Summary:

We are seeking a per diem Clinical Evaluator to join our Admissions department at Vernon Rehabilitation and Healthcare Center in Vernon, Connecticut. As a Clinical Evaluator, you will play a crucial role in assessing the nursing needs of our patients and ensuring the delivery of high-quality care. This is a multi-facility role, working remotely, with support needed mostly on weekends.

Responsibilities:

  • Assess the nursing needs of patients and develop individualized care plans

  • Conduct thorough evaluations of patients' medical histories, mental and physical status, and rehabilitation potential

  • Collaborate with healthcare teams to ensure comprehensive care and planning

  • Maintain accurate and detailed records of patient assessments, care plans, and progress

  • Provide education and support to patients, families, and caregivers

Requirements:

  • Registered Nurse or Licensed Practical Nurse licensed in the State of Connecticut with at least three (3) years' experience in an acute care setting or Long Term Care experience preferred

  • Minimum of two (2) years marketing, admission, and/or clinical evaluator experience

  • Interest in the nursing needs of the chronically and/or acutely ill, with knowledge of the affect on the elderly

  • Ability to read, write, and comprehend English; ability to follow oral and written instructions

  • Strong written, verbal, analytical, and interpersonal skills

  • Proficiency with MS Office suite (e.g., Outlook, Word, Excel)

  • Ability to work hours as scheduled based on the requirements of the position assignment
