Weekend Baylor Program, Registered Nurse (RN), Home Care, Emerson Health, Concord, MA
We are seeking a compassionate and skilled Registered Nurse (RN) to join our home care team through our Weekend Baylor Program. This role is ideal for nurses looking to maintain a consistent weekend schedule while delivering high-quality, patient-centered care in the comfort of patients' homes.
Schedule - Weekend Baylor Program:
  • 24 hours/week - Friday, Saturday, Sunday
  • 8:00 AM - 4:30 PM

About the Role:
The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes.
The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards.
Key Responsibilities:
  • Perform comprehensive patient assessments in the home setting
  • Develop and implement individualized care plans and goals
  • Provide skilled nursing care and treatments according to physician orders
  • Educate patients and families on disease management, medications, and care plans
  • Monitor patient progress and adjust care plans as needed
  • Document care accurately in the electronic medical record

Minimum Qualifications:
Education:
