Job description

Responsible for coordination of care for the residents living at Water's Edge Care Center. Consults with staff, physicians, management team on nursing problems and interpretation of facility policies to ensure resident needs are met. Monitors staff activities and care provided to seek out opportunity to enhance the quality of care provided to the resident with an emphasis on person centered care.
  1. Initiates and implements the policies and procedures that govern the Nursing Services in collaboration with the interdisciplinary team.
  2. Assist in the completion of needed assessment for admission, reviews the 48 hour CP and makes changes if needed, assist in completion of discharge paperwork, medication reconciliation, and identifying potential discharge needs in collaboration with therapy/social services.
  3. Demonstrates the knowledge of the geriatric resident and evaluates licensed nurse's abilities to assess and reassess residents. Completes required competencies timely, educate staff when it is needed
  4. Coordinates and directs resident care by reviewing the resident's care plan and observes for proper
    implementation of the care plan by the nursing staff for that unit. Makes changes to the Plan of Care as needed and provides information of changes to other staff members
