The Care Manager RN provides professional, comprehensive, patient-centered care management services for at-risk patients in an acute care environment. This role focuses on coordinating care across the continuum to ensure quality outcomes, appropriate resource utilization, and safe discharge planning.
Key responsibilities include:
• Patient assessment and care planning
• Coordination of care across multidisciplinary teams
• Resource utilization management and review
• Discharge planning and transitions of care

Qualifications:
• Associate Degree in Nursing (ADN) or nursing diploma
