This position is fully remote, allowing for flexibility while you work from anywhere in Texas, supporting clients in our community health clinic setting. You will primarily interact with seniors and their families via telehealth sessions, phone calls, and virtual meetings. The work hours are structured for a regular full-time schedule from Monday to Friday, 8:00 AM to 4:30 PM, with potential flexibility for evening or weekend hours to meet patient needs.

• Conduct in-depth psychosocial assessments to evaluate the emotional, mental, and social needs of seniors and their families.
• Collaborate with clients to develop personalized care plans that reflect their unique goals and preferences, ensuring each plan is achievable and client-centered.
• Provide ongoing counseling and emotional support, facilitating coping strategies for life transitions such as aging, loss of independence, or relocation to assisted living.
• Advocate for clients' rights and access to essential community resources, ensuring they receive the necessary support and services.
• Maintain a close working relationship with an interdisciplinary team of healthcare professionals, including nurses and geriatric specialists, to coordinate care and improve patient outcomes.
• Conduct regular virtual follow-ups and home visits when necessary to monitor clients' well-being and adjust care plans as needed.
• Educate clients and families about available resources, support groups, and health programs that can enhance social interaction and improve quality of life.
• Document all client interactions accurately and promptly, maintaining compliance with regulatory and ethical standards.

This role specifically targets the senior demographic, focusing on their unique psychosocial challenges. Through your expert guidance and compassionate support, you will help clients navigate complex emotional landscapes and advocate for their needs within the healthcare system and beyond. • Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant social work experience, ideally focused on geriatric care or within a community health setting.
• Strong foundational knowledge in social work principles, including those specifically related to aging issues and available community resources.
• Excellent skills in assessment, counseling, and problem-solving, enabling effective intervention and support.
• Empathetic and compassionate approach to working with seniors and their families, with the ability to build rapport quickly.
• Proficient communication skills to collaborate effectively with an interdisciplinary care team.
• Familiarity with electronic health record (EHR) systems and standard office software applications.

• Experience working with a diverse range of senior clients and their families, addressing various psychosocial, emotional, and behavioral issues.
• Previous involvement in community outreach programs or resources for aging populations.
• Knowledge of local community services and support networks specific to the San Antonio area.
