Speech-Language Pathologist (SLP)

Rochester, NY 14615

Job Summary:

Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in Rochester, NY. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Requirements:
  • Active NY State Speech-Language Pathologist License — required
  • Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
  • Basic computer literacy for documentation and communication
  • Strong clinical and interpersonal communication skills
  • Ability to work independently in a home health environment
  • Reliable transportation for home visits preferred
Job Responsibilities:
  • Evaluate patients and develop individualized treatment plans based on patients’ needs and goals
  • Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
  • Maintain accurate and timely documentation of patient care
  • Collaborate with the clinical team to ensure coordinated and effective treatment
  • Communicate effectively with office staff, patients, and caregivers
  • Collaborate with healthcare professionals, including speech therapy assistants, nurses, and physicians, to ensure comprehensive care
