Schedule: FT | M-F | 8am - 5pm

Join a collaborative, patientcentered rehab team where your clinical expertise and coordination skills truly make a difference. We're seeking a Case Manager to guide patients and families through the rehabilitation journey, ensuring seamless communication, effective care planning, and smooth transitions.

How you'll contribute:

  • Lead interdisciplinary care coordination and discharge planning
  • Identify barriers to progress and help develop solutions with the care team
  • Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
  • Facilitate weekly team conferences and family meetings
  • Communicate patient progress and discharge plans to caregivers
  • Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
  • Support additional departmental needs as assigned

Qualifications and requirements:

  • Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
  • 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
  • Strong communication, organization, and collaboration skills
  • Proficiency in Microsoft Office
  • Case Management or Rehab Nursing certification (CCM, ACM, RNBC, ARN); preferred
