Job Duties:
  • Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
  • Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
  • Document all services provided.
  • Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
  • Report changes in patient condition and needs to RN

Qualifications:
  • Current Arkansas Licensed Practical Nurse (LPN) license required
  • Minimum 1 year clinical experience as an LPN
  • Current CPR Certification
  • Compassionate and great time management skills
  • Reliable transportation to conduct patient visits at their place of residence or facility
