• Conduct detailed assessments of the psychosocial needs of senior clients, taking into account emotional, mental, and social factors affecting their well-being.
• Develop tailored care plans in partnership with clients and their families, ensuring that their unique goals and preferences are at the forefront.
• Offer compassionate counseling and emotional support to clients, assisting them in navigating significant life changes such as health concerns or loss of independence.
• Advocate for clients' rights, helping them access community resources and services designed to support their care and enhance their quality of life.
• Collaborate with a multidisciplinary team, including nursing staff and other healthcare professionals, to coordinate comprehensive care strategies.
• Regularly visit clients (virtually, as applicable) to monitor their well-being and assess living conditions, addressing any concerns or safety issues promptly.
• Facilitate open communication and mediation among family members involved in the care of senior clients, ensuring that their needs are respected and understood.
• Educate clients and their families on available community resources, including support groups and programs that promote social engagement.
• Maintain thorough documentation of all client interactions, care plans, and assessments to ensure compliance with best practices and regulatory standards.
• Stay informed about current best practices in social work and related policies, ensuring adherence to ethical guidelines in all interactions.

The primary focus of this role is to serve the senior population within the Tallahassee community. You will engage with elderly clients who may experience a range of challenges, including mental health issues, isolation, health-related concerns, and the emotional impact of aging. Your work will be pivotal in ensuring that these clients receive the support they need to thrive.

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of experience in social work, ideally within a community center or setting focused on geriatric care.
• In-depth knowledge of social work principles, particularly as they relate to aging and elderly care.
• Exceptional assessment and counseling skills, with a proven ability to empathize and connect with clients.
• Strong communication and interpersonal skills, fostering collaboration with team members and clients alike.
• Proficiency with electronic health records (EHR) systems and related technology.

• Previous experience working with seniors in a remote capacity, utilizing telehealth tools.
• Familiarity with community resources available to seniors and their families.
• Demonstrated ability to develop care plans that effectively address psychosocial needs.
