Work EnvironmentThis role is primarily remote, which allows for flexible scheduling to accommodate the unique needs of our clients and their families. The work setting involves regular interaction with clients through virtual consultations as well as on-site visits as needed. Our dedicated interdisciplinary team includes healthcare professionals such as nurses, caregivers, and chaplains, all collaborating to ensure a holistic approach to care.
Key Responsibilities- Conduct comprehensive psychosocial assessments of seniors, focusing on emotional, social, and mental health factors that affect their quality of life.
- Work collaboratively with clients and their families to develop and implement personalized care plans designed to meet specific needs and preferences.
- Provide one-on-one counseling sessions, offering emotional support as clients navigate life transitions and the process of terminal illness.
- Serve as an advocate for clients by facilitating access to community resources and services that enhance their care experience.
- Lead interdisciplinary team meetings to discuss ongoing cases, ensuring coordinated and effective care delivery.
- Regularly visit clients to monitor their well-being and living conditions, addressing any emerging concerns or safety issues.
- Educate families on available community resources and support services, helping them engage in their loved one's care.

Qualifications- A Master’s degree in Social Work (MSW) from an accredited institution.
- Current and valid licensure as a Licensed Clinical Social Worker (LCSW).
- At least two years of relevant social work experience, particularly in hospice, palliative care, or geriatric settings.
- Excellent assessment, counseling, and communication skills to effectively support clients and collaborate with healthcare professionals.
- Strong understanding of social work ethics, principles, and available community resources specific to elder care.
- Familiarity with Electronic Health Record (EHR) systems and proficiency in digital communication tools.
- Demonstrated experience in providing care in hospice or home health settings.
- Training or certification in grief counseling or elder care.
- A proven track record of effective collaboration within interdisciplinary teams.
The competitive salary for this position ranges from USD 60,000 to 100,000 annually, depending on experience and qualifications.
