• Conduct comprehensive psychosocial assessments to gather detailed information about patients' mental health concerns, strengths, and resources.
• Provide individualized therapy, including one-on-one, family, and group sessions, utilizing evidence-based treatment modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop and execute personalized treatment plans with clearly defined goals and interventions, updating them as necessary based on patient progress and feedback.
• Collaborate regularly with a multidisciplinary team of psychiatrists, psychologists, and nursing staff to ensure integrated care and support for all patients.
• Facilitate crisis intervention and offer risk assessments to identify and address safety concerns effectively.
• Maintain meticulous and timely documentation in compliance with all ethical, legal, and organizational standards.
• Offer and lead group therapy sessions, psychoeducation classes, and support groups, promoting shared experiences and healing.

Our client population primarily consists of individuals receiving hospice care, including those coping with terminal illnesses and their families. You will address a diverse range of mental health issues, including grief, anxiety, depression, and adjustment disorders, facilitating healing and resilience at a critical time in their lives. Your role is integral to providing emotional support while ensuring dignity and respect for each client’s individual journey.

• A Master’s degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of experience in a mental health or outpatient setting, particularly within hospice or palliative care environments.
• Strong knowledge of evidence-based therapeutic modalities and treatment approaches.
• Excellent interpersonal and communication skills, with a demonstrated ability to assess and respond to client needs.

• Experience working within a multidisciplinary team in a healthcare setting, particularly with terminally ill patients.
• Proficiency in electronic health record (EHR) systems for effective documentation and patient management.
• Prior experience in crisis intervention and managing complex cases.
