Job description

Position Overview
We are seeking an experienced (2-3+ years) Primary Care Nurse Practitioner (NP) to join two busy, well-established outpatient Family Medicine practices serving both pediatric and adult patients. This is a full-time hybrid opportunity with two days per week providing patient-facing care in clinic and three days per week working remotely.
Remote responsibilities will include telehealth visits and in-basket coverage as needed. This position is 100% outpatient and offers outstanding clinical and administrative support within a collaborative primary care environment.
Qualified candidates must be board certified, eligible for licensure in the State of New York, and have prior experience as a Nurse Practitioner in family medicine, primary care, pediatrics, internal medicine, or a related outpatient setting. This opportunity is not open to Physician Assistants or new graduate applicants.
This role will support both the Clinton Street and Windsor Primary Care offices and will include:
2 days per week of patient-facing care at Clinton Street
3 days per week of telehealth visits supporting both Clinton Street and Windsor
Patient-facing clinic days will be scheduled 8:00 AM - 5:00 PM
Telehealth days may require some flexibility in hours based on patient and practice needs

Annual Compensation Range:
$131,900 - $155,200
The ideal candidate will be comfortable managing acute and chronic conditions across the lifespan, performing preventive care visits, ordering and interpreting diagnostic testing, developing treatment plans, prescribing medications within scope of practice, documenting care in the electronic medical record, and communicating effectively with patients, families, physicians, and the broader care team. Strong clinical judgment, independence, and proficiency with telehealth and electronic in-basket workflows are essential.
Education/Experience
Minimum Required:
  • Master of Science in Nursing (MSN) degree
  • Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number
  • Successful completion of an approved Nurse Practitioner program
  • Three (3) years of professional nursing experience in a clinical setting

Preferred:
  • Doctor of Nursing Practice (DNP)

License/Certification
Minimum Required:
  • Current NYS Registered Professional Nurse License
  • American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) certification
  • Current NYS Certified Nurse Practitioner (CNP) license and certification with New York State Education Department
  • Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
  • Certified Prescriptive Privilege in NYS

Preferred:
  • Advanced Cardiovascular Life Support (ACLS) Certification
  • Possession of Bureau of Narcotics and Dangerous Drugs (BNDD)Number

Salary Range: $131,900 - $155,200 base salary
