This full-time role offers an annual salary ranging from USD 70,000 to 100,000, depending on experience. You will have access to a variety of resources to assist you in providing the best possible care from the comfort of your home. Our flexible work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability based on client needs. • Perform comprehensive assessments to evaluate the psychosocial needs of seniors, integrating emotional, mental, and social factors into your care approaches.
• Engage collaboratively with clients and their families to co-develop personalized care plans that reflect specific goals, preferences, and support systems.
• Provide compassionate counseling and emotional support, guiding clients through significant life transitions such as chronic illness or end-of-life decisions.
• Advocate for clients' rights, facilitating access to community resources, healthcare services, and support programs.
• Coordinate with interdisciplinary teams, including nurses, caregivers, and mental health professionals, to ensure comprehensive and cohesive care delivery.
• Conduct regular virtual check-ins to monitor clients' emotional and physical well-being, making adjustments to care plans as needed.
• Facilitate difficult conversations and conflict resolution among family members regarding the care of their loved ones.
• Educate clients and their families about relevant community resources, available support groups, and programs aimed at enhancing their overall quality of life.
• Maintain thorough documentation of all client interactions, assessments, and progress notes to ensure continuity of care and compliance with ethical standards.
• Keep abreast of the latest social work practices, regulations, and ethical guidelines impacting service delivery within our hospice framework.

This role primarily focuses on senior clients who are navigating the complexities of hospice care. As a social worker, you will be instrumental in addressing the emotional and psychological needs that often accompany serious health challenges. Your work will directly contribute to improving patients' emotional health, providing them with tools to achieve greater comfort and dignity during their most vulnerable days.

• A Master’s degree in Social Work from an accredited institution, ensuring a strong foundation in social work practices.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW), validating your professional credentials.
• A minimum of two years of relevant experience in a therapeutic role, ideally within a community or geriatric setting, enhancing your understanding of client needs.
• Proficient knowledge of social work principles, particularly relating to aging, mental health, and available community resources.
• Exceptional interpersonal skills, demonstrated empathy, and an ability to connect with clients from varying backgrounds and circumstances.

• Experience in hospice or palliative care settings, providing an understanding of the unique challenges faced by clients and families during the end-of-life phase.
• Familiarity with telehealth platforms and electronic documentation systems to facilitate effective virtual service delivery.
• Previous experience working as part of an interdisciplinary team in healthcare, which will enhance your collaborative skills in a remote work setting.

This full-time position offers a competitive annual salary between USD 70,000 and 100,000, based on experience.
