Choice Physical Therapy & Wellness, Inc is seeking a dedicated and experienced Licensed Mental Health Therapist (LMFT, LPC, LCSW) to join our multidisciplinary treatment team. As a remote-based therapist working for Choice Physical Therapy & Wellness, you will have the flexibility to work from home while remaining connected with your team through digital platforms. Your daily responsibilities will include:

• Conducting individual therapy sessions with patients, focusing on their specific mental health needs and treatment goals under the guidance of nursing staff.
• Providing continuous observation of patients, documenting their progress, and ensuring that all treatment interventions align with their therapeutic objectives.
• Orienting patients and their families to the treatment process, addressing questions or concerns, and fostering a therapeutic alliance.
• Assisting patients with personal needs as necessary to enhance their overall comfort and recovery experience.
• Writing detailed progress notes that accurately reflect patient responses, developments, and progress toward goals, complying with departmental standards.
• Staying informed of assigned patients’ treatment goals and advocating for additional services or adjustments when necessary to ensure optimal care.

You will work with a diverse patient population, which includes individuals experiencing anxiety, depression, trauma, and other mental health conditions. Your expertise will help guide these patients in their recovery journeys, leveraging your skills to provide tailored support and guidance that fosters positive outcomes. The comprehensive support you provide will be integral to ensuring each individual feels heard, understood, and empowered throughout their treatment.

To be a successful candidate for this role, you must meet the following qualifications:

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling.
• Active license (LMFT, LPC, LCSW) issued by the state board. Limited or provisional licenses will not be accepted.
• A minimum of 3 years of experience working in mental health or clinical settings, demonstrating strong diagnostic and clinical skills.
• Strong analytical abilities to evaluate patients, develop treatment plans, and make informed decisions regarding care.
• Outstanding interpersonal skills, demonstrating empathy and compassion when engaging with patients and their families.
• Proficiency in computer use, including electronic health record systems for documentation.

Experience in an outpatient mental health setting is preferred. Familiarity with evidence-based therapeutic modalities, treatment planning methodologies, and the ability to work collaboratively within a team are essential for delivering comprehensive patient care in our integrated approach.
