Work Environment

This position offers a full-time remote work opportunity, allowing you to conduct your sessions from the comfort of your home while still being an integral part of our community health clinic team. Our operational hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client needs and availability. Flexibility in scheduling is vital to ensure that we provide continuous care and support to our clients.

Key Responsibilities
  • Conduct in-depth assessments of clients' psychosocial needs, focusing on emotional, mental, and social factors that may impact their overall health.
  • Collaborate with clients and their families to develop personalized care plans that align with their unique goals and preferences, ensuring their voices are central to the care process.
  • Provide counseling and emotional support to clients, guiding them through life transitions and helping them navigate various challenges with resilience.
  • Advocate passionately for clients' rights, ensuring they have access to vital community resources and necessary services.
  • Work interactively with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care that enhances clients' quality of life.
  • Conduct regular virtual check-ins to monitor client progress, assess living conditions, and address any safety or wellbeing concerns.
  • Facilitate effective communication and conflict resolution among family members involved in a client’s care, fostering a collaborative approach to support.
  • Educate clients and families about available resources, support groups, and programs to enhance their social engagement and quality of life.
  • Maintain meticulous records of client assessments, care plans, and interactions to ensure compliance with healthcare standards and facilitate ongoing support strategies.
  • Stay current with social work practices, policies, and regulations to maintain the highest ethical standards in all client interactions.

Qualifications

Preferred Experience

Compensation and Benefits

This full-time position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
