• Conduct thorough psychosocial assessments for patients and their families at the point of admission and periodically thereafter to determine the necessary support services while addressing any barriers to care.
• Collaborate with interdisciplinary healthcare teams to develop comprehensive psychosocial treatment plans, facilitating referrals to additional specialty services, entitlement programs, and ensuring a continuum of care through meticulous documentation.
• Engage patients and families in navigating the emotional impacts of illness, providing tailored supportive counseling, grief counseling, and crisis intervention as needed.
• Manage an independently assigned caseload of patients with varying clinical needs, ensuring a timely and effective therapeutic relationship.
• Participate in team meetings and case conferences, proactively identifying patients with psychosocial vulnerabilities.
• Deliver psychoeducation to enhance patients’ and families’ understanding of diagnoses, treatment options, and prognosis—particularly aiding in transitions from youth to adult services.
• Contribute to quality assurance initiatives aimed at improving social work services and patient outcomes through active participation in policy reviews and development.
• Provide clinical supervision and guidance to MSWs and student interns, ensuring a nurturing learning environment.
• Maintain thorough electronic medical records in accordance with both departmental and health system policies while preparing detailed reports and hand-offs for continuity in patient care.

You will work closely with a broad demographic of clients, including adults, seniors, children, and adolescents, each facing unique mental health challenges. Our diverse clientele requires a counselor who can tailor their approach to meet individual needs, whether it be addressing anxiety, depression, grief, or other psychological issues. • Active and unrestricted licensure as an LCSW, LMFT, or LPCC in California.
• A Master’s degree in a behavioral science or related field is required from an accredited institution.
• Knowledge of effective assessment, interviewing, and counseling techniques applicable to various psychological needs.
• Demonstrated ability in crafting written case narratives and reports that clearly outline patient progress and insights.
• Strong background in social work theories and methodologies to drive effective patient outcomes.

• Experience in a telehealth setting preferred, demonstrating familiarity with technology and virtual client engagement.
• Prior involvement in multidisciplinary team settings would be advantageous for fostering collaborative care.
• Supervisory experience overseeing interns or junior staff in a clinical environment is a plus.
