• Conduct thorough psychosocial assessments through interviews, observations, and a review of patients' medical records to evaluate the emotional, social, and environmental determinants of their recovery.
• Deliver comprehensive counseling services to patients and their families, focusing on adjustment issues, emotional resilience, and coping strategies to facilitate positive outcomes.
• Collaborate closely with our rehabilitation team—including physical therapists, occupational therapists, and speech-language pathologists—to design customized treatment plans that address each patient's unique psychosocial needs.
• Assist patients in navigating community resources such as occupational rehabilitation programs, support groups, and housing assistance, ensuring a holistic approach to their recovery.
• Foster patient education by guiding individuals and their families on availability and access to post-rehabilitation services and resources.
• Contribute to the discharge planning process by preparing patients for transitions to home or other care settings, ensuring continuity of care and support.

At Imaginative Insights, you will work with a diverse patient population comprised of individuals overcoming various conditions, including stroke recovery, spinal cord injuries, orthopedic surgeries, and neurological disorders. Our focus is on maximizing patient independence and enhancing their emotional well-being, recognizing the profound connection between mental and physical recovery.

• Master's degree in Social Work from an accredited program.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Strong understanding of psychosocial factors influencing recovery, including adjustment issues, grief, and coping mechanisms.
• Exceptional communication and interpersonal skills, enabling you to build effective relationships with patients and collaborate with interdisciplinary teams.

• Previous experience in a rehabilitation or clinical medical setting is highly desirable, enhancing your ability to navigate complex patient cases.
• Familiarity with community resource networks and best practices in rehabilitation counseling.
