Project Reach Psychotherapy is seeking a dedicated and professional Licensed Certified Social Worker (LCSW-C) to join our team in providing exceptional mental health support within our hospice center located in Pomfret, Maryland. At Project Reach Psychotherapy, you will work primarily in our hospice center while having the flexibility to provide services remotely. This hybrid model supports both in-person interactions with patients and virtual counseling sessions, enhancing your ability to connect with clients in their preferred setting. • Provide high-quality psychotherapy services to individuals, couples, and families navigating grief and emotional distress related to terminal illnesses.

• Conduct thorough intake assessments to understand the unique situations of each client and develop comprehensive, individualized treatment plans that prioritize their emotional and psychological needs.

• Maintain meticulous clinical documentation using electronic health record (EHR) systems to ensure compliance with all regulatory and ethical standards, as well as facilitate effective communication with other healthcare providers.

• Engage in regular supervision, consultation, and multidisciplinary team meetings to continuously improve the quality of care provided to our clients.

• Adhere to Maryland state licensing regulations and uphold the highest ethical standards in all interactions with clients and team members.

Your primary focus will be on clients in a hospice setting, where you will work with patients and their families who are facing serious illnesses and end-of-life transitions. You will support them in managing anxiety, grief, and interpersonal conflicts that may arise during this challenging time. Your role will be instrumental in helping clients cultivate a sense of peace and understanding as they navigate their circumstances.

• Required: Active LCSW-C license in the state of Maryland.

• A Master’s degree in Social Work from an accredited program is essential.

• Demonstrated strength in clinical skills with a commitment to providing culturally responsive care.

• The ability to work independently and effectively manage a diverse caseload of clients.

• Proficient with electronic health record systems or willingness to learn required digital tools.

• At least 2 years of clinical experience in providing psychotherapy, particularly in hospice or similar settings.

• Experience conducting family or couples therapy, with a focus on end-of-life issues.

• Advanced skills in diagnosing and treating mental health disorders, understanding the relationship between physical health and emotional wellness.
