JOB DESCRIPTION:

SUMMARY OF POSITION

The Steven A. Cohen Military Family Clinic offers a variety of services to post-9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members.The Master's Level Intake Coordinator is the point of contact for all potential clients requesting services from the Steven A. Cohen Military Family Clinic at Centerstone. This position is responsible for managing and executing referral screening intake appointments in via in-person, telephonically, or by use of tele-video with post-9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members. The Intake Coordinator will administer and complete appropriate initial paperwork, psychometric screeners, diagnostic assessment, risk screening and health record documentation to support ongoing treatment. The Intake Coordinator will work with a multi-disciplinary team of clinic staff for case assignments, consultations and care coordination.

The Master's Level Intake Coordinator will additionally provide clinical treatment to the military population served via a caseload of clinical cases. Additionally, the Intake Coordinator will provide clinical treatment to post-9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members. The Intake Coordinator will provide evidence-based therapy to individuals, couples, families and groups.The Steven A. Cohen Military Family Clinic has a commitment to training, consultation and proficiency in Cognitive Behavioral Therapy, Cognitive Processing Therapy, and Prolonged Exposure, through which the Intake Coordinator will deliver short-term treatment, targeted treatment. The Intake Coordinator will support the Centerstone Mission, Vision and Values in delivering care that changes people's lives and is expected to communicate clearly and appropriately; providing excellent customer service. Adherence to applicable policies and procedures is expected. Regular attendance and punctuality is required.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Conducts comprehensive in-person, televideo and telephonic intake and referral screeners with post 9/11 Veterans, Active-Duty Service Members, Reservists, National Guard and their family members.
  • Complete a thorough diagnostic and risk assessment using evidence-based screening measures, and safety plan as appropriate with potential clients reporting risk.
  • Document detailed intakes in the electronic health records (EHR) system; maintain comprehensive client referral tracking system; prepare reports as requested by the Assistant Director and/or Clinic Director.
  • Provides appropriate referrals and follow-up as necessary.
  • Performs and documents patient's screenings, psychological assessments, progress and referrals for service members, veterans and military family members.
  • Engages in regular training and consultation to engage in evidence-based, measurement informed care for clients.
  • Provides individual, couples, groups, and family psychotherapy to clients with a focus on delivering evidence based treatments under supervision of clinical supervisor
  • Prepares and maintains confidential case records and documentation in the electronic health record.
  • Ensures confidentiality and controls access to sensitive information.
  • Collaborates with clinical team to perform risk management and crisis intervention services as necessary.
  • Develops network of trusted contacts within organizations in which to refer and coordinate care.
  • Assists veterans and their family members in navigating service providers across multiple systems of care
  • Attends weekly individual clinical supervision and weekly group clinical/admin supervision and administrative meetings
  • Provides direct services including treatment for Veterans and their families.
  • Collaborates and coordinates with the caregiver, physician, clinic director, case management and all other disciplines to provide the best care to patients as needed.
  • Consults with service providers and community partners on resource-related issues.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of and experience working with local social service, public health, and VSO organizations and resources.
  • Understanding of a range of treatment modalities to include child, couples, and family treatment is preferred.

QUALIFICATIONS

Education Level

  • Master's Degree in Social Work, Sociology, Psychology or related field required.
  • 1+ year's behavioral health experience required; 3+ years preferred. 2+ years in a customer service focused environment.

Years of Experience

  • One or more year's behavioral health experience required; three years preferred.
  • Two or more years in a customer service focused environment required.
  • Military background or prior experience in serving the military community and/or veteran administrative organizations preferred.
