We are looking for a director of facility partnerships who is committed to serving our patients with compassion and integrity. The Director of Hospital Relationships is a strategic leader responsible for developing, strengthening, and expanding hospital partnerships that support organizational growth, patient access, and exceptional care transitions across the post-acute continuum. This role serves as a trusted liaison between the organization and hospital partners, building executive-level relationships while identifying opportunities to increase collaboration, referral partnerships, market presence, and long-term business value. The Director will develop and execute hospital partnership strategies, establish scalable processes and best practices, and collaborate cross-functionally with clinical, operations, sales, and leadership teams to ensure partnerships are aligned with organizational goals and the needs of patients, families, and healthcare providers. The role will proactively identify market opportunities, resolve partnership challenges, communicate organizational capabilities, and create meaningful alignment between hospital and hospice teams.
Success in this role requires a strong understanding of the hospice and post-acute care continuum, demonstrated healthcare leadership experience, exceptional relationship-building skills, and the ability to translate partnerships into measurable growth and improved patient outcomes. The Director will monitor partnership performance, analyze key metrics, forecast market and referral trends, and provide leadership with actionable insights to inform strategic decisions and future growth.

Qualifications
  • A heart to serve patients and families and a passion for providing the best possible care
  • EDUCATION: Minimum of Bachelor's Degree required in the field of study
  • EXPERIENCE: 5 - 10 years of leadership experience in a health care setting or in sales/marketing with demonstrated knowledge of every aspect of the post-acute care model. Hospice experience required
  • REQUIRED: Reliable transportation
