Licensed Practical Nurse

Job Description

The Licensed Practical Nurse (LPN) is responsible for assisting the PACT with ongoing, continuous care of one or more assigned panel of Veterans. This LPN is responsible for basic LPN duties as listed below and other duties as assigned:

  • Provide direct patient care.
  • Monitoring vital signs.
  • Observes, records and reports patient's response or changes in patient's condition.
  • Assisting the provider with exam/procedures.
  • Keeping Veterans advised of delay/wait times.
  • Reviewing daily clinic schedule for patient care needs.
  • Promptly recognizes and responds to urgent or emergent situations.
  • Checks daily equipment/supplies and restocks the exam rooms.
  • Collaborate with team to develop comprehensive health care plans and care management plans for patients.
  • Acts as patient advocate.
  • Assures patient understands content of visit and when to return to clinic.

Work Schedule: intermittent as needed, Monday-Friday 8am to 4:30pm
