Job description

Sentara Medical Group has an employment opportunity for a full time Nurse Practitioner or Physician Assistant to join its Palliative Care team on the campus of Sentara Princess Anne Hospital located in Virginia Beach, VA. The Nurse Practitioner or Physician Assistant is responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team.

Position Highlights:

  • Monday-Friday; 8:30a - 5:00p
  • Performing palliative care consultations and follow up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
  • Facilitating family meetings related to current medical decision making and advance care planning
  • Assisting with developing appropriate discharge plans with respect to established goals of care including close collaboration with Hospice for patients to ensure access and continuity of care.
  • Actively participating as a member of an interdisciplinary team to ensure that palliative care is provided in a manner which addresses pain and symptom management; psychosocial/spiritual support; and caregiver support.
