1 FTE Vietnamese Speaking & 1 PT Mandarin Speaking Available

Starting at $33.06 - $ 36.78/hour (D.O.E.) plus a $250 stipend bonus per month!

How YOU can make a difference

The Long-Term Care (LTC) Supervisor ensures frail seniors and individuals with disabilities live safely at home through high-quality personal care. Reporting to the LTC Service Director, you will manage a caseload of 80-100 clients and supervise 50-60 Home Care Assistants (HCAs). This role balances clinical advocacy, staff mentorship under a Union Collective Bargaining Agreement, and rigorous DOH regulatory compliance.

Home Care Supervisor Job Description

  • Client Intake & Advocacy:
    • Conduct initial home visits to orient clients to rights and policies.
    • Perform comprehensive safety inspections, and develop personalized care plans and emergency protocols.
  • Personnel Management:
    • Supervise and mentor 50-60 Home Care Assistants, including hiring, training, conducting performance reviews and managing corrective actions under the Union Collective Bargaining Agreement.
  • Case Coordination & Staffing:
    • Manage a caseload of 80-100 clients, matching them with appropriately trained HCAs, monitoring client satisfaction, and ensuring 24/7 coverage through a rotational on-call program.
  • Regulatory & Safety Compliance:
    • Ensure strict adherence to DOH licensing rules, LTC safety programs, and universal precautions while serving as a mandated reporter and community liaison.
  • Administrative Oversight:
    • Manage clients and HCA data entry, verify electronic visit records for accurate daily payroll and billing, and maintain organized digital and paper files.

Access to and protection of PHI/ePHI

  • Holds (primary) leadership role in creating a culture of privacy and security of Protected Health Information in HIPAA Covered Entity service area, including education of staff about their responsibilities for safeguarding privacy and security.
  • Authorizes level of staff access to PHI/ePHI to ensure staff can perform assigned job duties; regularly monitors access to ensure appropriate safeguarding of PHI/ePHI.

Job Conditions

This position requires the employee to work in an office environment where there may be interruptions and a moderate amount of noise from telephones and conversations among employees. Extensive use of computers and related office equipment is typical of this position.

Minimum Qualifications

  • Bachelor's Degree in Business, Health Care or Medically related field and four years of experience working in a fast-paced Health Care delivery system. (Experience may be substituted for Education; successfully completing intensive six-month on-the-job LTC Supervisor training at CCS may substitute for education).
  • Home Care Aide License, Nursing Aide Certification, or equivalent license
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills with the ability to work within systems
  • Ability to work as part of a team or independently
  • Previous work history in health services
  • Demonstrated ability to work with a diverse workforce
  • Car with valid Driver's License and automobile insurance
  • Pass required criminally history background check

Preferred Qualifications

  • Three years of experience supervising Home Care, Home Health, or Hospice program
  • Demonstrated knowledge of computer programs
  • Experience with Medicaid home care
  • Familiarity with the designated Service Community
