Job description

Health First is seeking an experienced Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) specializing in Heart Failure to support our expanding cardiology services in Brevard County, FL.

This dedicated outpatient clinic role is ideal for a seasoned clinician seeking a manageable patient volume, strong physician collaboration, and a focused clinical environment centered on comprehensive heart failure care.

Position Highlights

  • Manageable Workload: See 12-15 patients per day, allowing for high-quality, comprehensive patient interactions.

  • 100% Outpatient / Strictly Clinic: Work in a dedicated clinic setting focused on long-term disease management and post-hospital follow-up.

  • Strong Collaborative Support: Work closely with dedicated cardiologists to develop treatment plans, manage complex cases, and determine appropriate escalations.

  • Work-Life Balance: An answering service is available after hours for established patients only.

Key Responsibilities

  • Provide specialized clinical care for heart failure patients in an outpatient setting.

  • Conduct targeted post-hospital discharge follow-ups to support seamless transitions of care, optimize treatment, and reduce readmissions.

  • Manage complex chronic heart failure therapies and provide ongoing patient education.

  • Monitor patients' clinical status and adjust treatment plans as appropriate.

  • Collaborate directly with cardiologists on care plans, complex cases, and clinical escalations.

Qualifications

  • Active or eligible Florida APRN or PA license.

  • Minimum of 1-2+ years of clinical experience in cardiology or specialized heart failure management required.

  • Strong diagnostic and clinical decision-making skills in outpatient chronic disease management.

  • Excellent communication and collaboration skills.

  • Ability to independently manage complex patients while working effectively within a physician-supported care model.
