(Physician/MD qualifications required) Family Practice - Connecticut non-profit community based healthcare organization is seeking a family physician who can lead a healthcare team in providing comprehensive solutions that address complex health issues, providing individuals, families and communities with accessible, innovative care that encourages health, recovery and growth at all stages of life. The physician leads and works with the multi-disciplinary team to create integrated treatment plans that address the patient's physical and psychological needs and address the social determinants that impact health outcomes. provides whole person healthcare from infants to older adults in an integrated care setting. Takes lead role with APRNs and provides day to day clinical oversight in the absence of the Medical Director. Physician will diagnose and treat acute and chronic illnesses, provide routine health screenings and counseling on lifestyle changes in an effort to prevent illnesses before they develop. Physician will guide and coordinate all aspects of care as an engaged and active member of a multidisciplinary treatment team. Physician will work with the team to achieve the best possible patient outcome in the most cost-effective manner. Assists the department vice president in developing clinical protocols, continued quality improvement and provides recommendations regarding clinical workflow and process improvement. Working with the VP of Health Center Operations and Medical Director provides clinical oversight for the performance improvement activities of the health center including oversight and management of clinical performance outcomes.
