Opportunity Information
Position Overview
Sutter Senior Clinics are a part of the primary care continuum of care specifically for the care of vulnerable, value-based Medicare patients. These clinics manage sub-acute and chronic medical conditions, promote wellness and preventive health, and advocate for seniors-especially those with complex needs or from underserved communities. These clinics bridge clinical expertise with compassionate care, ensuring seniors achieve optimal health outcomes and quality of life.
The Senior Clinic Chief for Sutter Senior Clinics not only delivers comprehensive, high-quality, and patient-centered primary care for older adults, but this role, in partnership with the local administrative leader, is also responsible for the effective deployment of the day-to-day clinical operational model that allows for the holistic care that these clinics are meant to deliver.
As a core member of an interdisciplinary team, the Senior Care Chief ensures that the interdisciplinary process employed in the clinic through shared communication venues such as huddles and interdisciplinary team rounds allows all members of the care team to add in their valuable insight based on their expertise and knowledge of the patient. The Senior Care Chief is also responsible for ensuring that other senior clinic clinicians gain and maintain competency in the principles and application of the comprehensive assessment and longitudinal management practices that will be utilized in all Sutter Senior Clinics.
Key Responsibilities
  • In conjunction with the CMO, Senior Clinics, the Senior Care Chief ensures that the fidelity of the holistic clinical care model is maintained and enhanced
  • Responsible for fostering a patient-centric and outcomes-driven culture where the interprofessional model of care is valued by all members of the team through leadership in interdisciplinary team rounds, huddles and team meetings
  • Accountable for the financial and clinical outcomes of the clinic in partnership with the local administrative leader
  • Responsible for evaluating and coaching clinicians to build successful relationships with staff, patients and caregivers to create a culture of team-based care
  • Be a role model for care delivery in a population health, highly patient-centric, and team-based care model

Reporting:
  • 2 SHN sponsored FTE reports to CMO, Sutter Senior Clinics
    • Duties include but are not limited to:
      • Collaborating with the Sutter Senior Clinic CMO regarding staff oversight, staff and physician/APC education, Quality and VBC metrics.
      • Building relationships with GMG Ambulatory Chiefs and Chairs as well as individual physicians to empanel patients to the Sutter Senior Clinic.
      • Engagement with Hospital leadership, GMG Hospitalists, SNF Medical Directors to address utilization metrics (ED visits, inpatient days, SNF days etc).
      • Outreach to patients and the community at large regarding the clinic, it's services and structure.
      • Participate in Sutter wide leadership training, Senior Care specific education and training as well as development of protocols, documentations standards, IDT templates, and panel management practices.

  • 8 FTE Clinical reporting to the Med Director of VBC and Quality, and the IM Chair
    • Includes direct patient care, IDT meetings and coordination with hospitalists, specialists etc.

Qualifications
  • MD or DO degree from an accredited institution; board certified in Family Medicine or Internal Medicine
  • Geriatrics fellowship training is highly preferred
  • Openness to additional Geriatrics training
  • Valid and unrestricted state medical license
  • Minimum 2-5 years of experience required in a primary care/geriatric setting or geriatric fellowship completion
  • Prior experience in leadership in a primary care setting preferred
  • Familiarity and proficiency with electronic health record systems; comfort with utilizing Microsoft Office product
  • Familiarity with working in panel-oriented and team-based interdisciplinary clinical settings
  • Experience with value-based care models preferred
  • Demonstrated ability to influence and lead peers through change
  • Outstanding clinical diagnostic, communication, and patient engagement skills
  • Passion for working with seniors and their families in a collaborative, team-based environment

  • Starting base salary: $346,500
  • $50K incentive potential (via SHN) based on the clinic's performance on clinical, utilization and operational measures in alignment with system goals
  • Prorated APC supervision of $12K per1.0 FTE APC supervised
  • M-F traditional clinic hours
  • Shared call schedule with other Sutter Senior Clinic physicians and APCs. Expect no more than one week in three weeks
