Valiant Mind Counseling Center is seeking a dedicated Full-Time Licensed Clinical Social Worker (LCSW) to join our dynamic healthcare team. • Provide evidence-based psychotherapy and mental health services tailored to individual patients' needs, focusing on conditions such as anxiety, depression, and PTSD.
• Conduct comprehensive mental health assessments to evaluate emotional, behavioral, and social functioning.
• Collaborate with healthcare professionals to develop individualized, culturally competent treatment plans that reflect the patients' goals and preferences.
• Monitor and document patient progress accurately using electronic health records (EHRs), ensuring that treatment plans are updated based on observable outcomes and feedback.
• Maintain a compassionate yet professional demeanor, creating a secure space for patients to explore their thoughts, feelings, and behaviors.

You will primarily work with patients in hospice care who are experiencing complex emotional and psychological challenges as they navigate end-of-life issues. This role involves not only supporting patients but also providing guidance to their families, helping them cope with grief and facilitate discussions about care preferences and communication.

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Exceptional communication and interpersonal skills, crucial for establishing rapport with patients and collaborating effectively within a multidisciplinary team.
• Strong clinical acumen, including the ability to develop and execute evidence-based treatment strategies.
• Proficiency in using EHR systems and other professional software to manage patient information and coordinate care activities.

• Prior experience in a hospice or palliative care setting is highly desirable.
• Experience working with diverse populations and addressing culturally-specific needs in mental health care.
• Familiarity with telehealth practices and remote patient engagement techniques would be advantageous.
