Job description

POSITION SUMMARY:

The Transfer Center Nurse is responsible for coordinating patient admissions and transfers between Boston Medical Center Health System facilities. This role ensures a smooth, efficient, and safe transition of patients by acting as a liaison among patients, healthcare providers, and receiving facilities. The position requires strong clinical knowledge, organizational skills, and the ability to communicate effectively with diverse stakeholders.

Schedule: Per Diem: Day, Evening, Nights, Weekends

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Coordinate patient admissions from external and internal sources, verifying clinical and insurance information to facilitate timely processing.

  • Manage patient transfer requests, prioritizing based on medical urgency, bed availability, and resource allocation.

  • Serve as the primary point of contact for physicians, nurses, case managers, and sending or receiving facilities throughout the admission and transfer process.

  • Review patient medical records and documentation to ensure accuracy, completeness, and compliance with hospital policies and regulatory requirements.

  • Facilitate bed assignments and communicate status updates to involved parties.

  • Provide clinical assessment and triage support during admission and transfer processes to ensure patient safety and an appropriate level of care.

  • Collaborate with case management and social work teams to address complex admission or transfer situations.

  • Maintain accurate records of admissions and transfers for reporting and quality assurance purposes.

  • Participate in initiatives aimed at improving transfer center operations, patient flow, and overall patient experience.

  • Provides coverage for the Off-Shift Nurse Supervisor as needed.

JOB REQUIREMENTS

EDUCATION

  • Nursing degree: Diploma, ASN or BSN (preferred), Ability to obtain BSN within 4 years

  • Bachelor's degree preferred

  • If you are a RN with a Diploma or ASN who's not yet completed their BSN, you must commit to doing so within your first 4 years of employment.

EXPERIENCE

  • Minimum of 5 years clinical nursing experience

  • Previous experience working in a transfer center or admissions department preferred.

  • Previous experience preferably in emergency, critical care, or intermediate care.

CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED:

  • Requires license to practice professional nursing as a Registered Nurse in the Commonwealth of Massachusetts.

KNOWLEDGE AND SKILLS:

  • Decision Making: Demonstrates ability to make sound and timely decisions that are guided by precedents, policy & procedure and overall goals and objectives, is able to choose the best course of action based on analysis of available information, and can develop alternative courses of action

  • Problem Solving: Demonstrates ability to address a wide variety of clinical, operational and social issues that arise by analyzing the situation and applying critical thinking skills to implement a solution

  • Communication Skills: Demonstrates the ability to articulate thoughts and present complex clinical and operational information using oral, written, and non-verbal communication skills, able to employ active listening skills to gain clarity, having the ability to gather and deliver information in person, in writing, and electronically/digitally

  • Familiarity with hospital admission processes, insurance verification, and regulatory compliance

  • Strong clinical assessment skills and knowledge of patient care standards

  • Excellent organizational, communication, and interpersonal skills

  • Computer/Technology Skills: Demonstrates ability to access and enter data from EPIC EHR, excel, power point, Microsoft word, Microsoft outlook, web based shared drives

  • Teamwork: Demonstrates the ability to work with others toward a shared goal, share responsibility and rewards with team members, having the ability to recognize combined actions of the group leads to overall success of the team

Compensation Range:

$43.03- $62.50

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
