• Conduct thorough psychosocial assessments for patients and their families to evaluate emotional and social factors affecting their health-related situations.
• Develop, implement, and assess personalized intervention plans tailored to meet the unique needs of each patient, utilizing evidence-based practices.
• Facilitate collaborative planning and communication among the healthcare team, ensuring that patients and families are actively involved in planning their care.
• Manage comprehensive discharge planning by coordinating with medical professionals, third-party payers, and community resources to arrange appropriate follow-up care.
• Provide educational resources and counseling for patients and families to assist them in coping effectively with illness and life changes.
• Utilize crisis intervention skills and provide individual, group, and family therapy sessions tailored to the unique needs of oncology patients and their families.
• Serve as a knowledgeable resource for staff and contribute to the improvement of professional practices through education and training.
• Participate in on-call rotations to respond to after-hour crisis situations, demonstrating a commitment to continuous support for our patients.
• Present at educational conferences as a representative of Urban Arc Psychiatry Clinic and an expert in psychosocial oncology practices to external audiences.
• Engage in community services through outreach, research, and volunteer efforts, enhancing the well-being of cancer survivors and promoting available resources.

This role specifically focuses on patients facing mental health challenges related to their medical diagnoses, including those undergoing treatment for cancer and other chronic conditions. The Social Worker will address the unique psychosocial impacts of illness, guiding patients and families through their emotional and practical challenges while promoting mental health rehabilitation and quality of life improvements.

• Must hold a Bachelor's degree in Social Work from an accredited institution; a Master's degree or equivalent is preferred.
• Possess a valid and unrestricted LCSW (Licensed Clinical Social Worker) certification in the state of Illinois.
• Demonstrated knowledge of mental health practices, crisis intervention, and the needs of oncology patients.
• Exceptional interpersonal skills and the ability to work effectively in a virtual team environment.

• Experience in a clinical setting, particularly in oncology or chronic illness management.
• Previous remote work experience is a plus, highlighting adaptability in a virtual workplace.
• Familiarity with community resources and the ability to connect patients with appropriate external support services.

The compensation for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
