Responsibilities:

  • Answer inbound calls, emails, and web inquiries from prospective clients, family members, and referral sources in a timely, professional, and compassionate manner.

  • Provide clear information about Synergy HomeCare services, care options, pricing structures, and processes.

  • Conduct initial intake interviews to gather demographic, medical, functional, and social information needed to determine service needs.

  • Enter and update all client information accurately in the client management system.

  • Schedule in-home assessments, consultations, and follow-up calls for the Care Manager or Director of Operations.

  • Coordinate with internal team members to ensure seamless transition from initial intake to care planning and service start.

  • Training Follow-ups, Hiring Support, and Employee Life Cycle.

Requirements:

  • High school diploma or equivalent.

  • 1-2 years of experience in home care, healthcare, senior services, or customer service role involving direct client contact.

  • Strong phone and interpersonal communication skills; comfortable talking with seniors and families in stressful or emotional situations.

  • Excellent organizational skills with strong attention to detail and accuracy.

  • Proficient with computers and common office software (e.g., Microsoft Office, electronic client management systems, CRM - AxisCare, Training Tools - Care Academy).

  • Ability to multi-task, prioritize, and work in a fast-paced environment.

  • Demonstrated professionalism, discretion, and respect for confidentiality (HIPAA compliance).

  • CAN START ASAP

Preferred:

  • Experience in home care, home health, hospice, senior living, or related healthcare environment.

  • Knowledge of non-medical home care services, long-term care insurance, and private pay arrangements.

  • Bilingual skills a plus.
