• Conduct thorough assessments of clients to identify their social, emotional, and psychological needs, utilizing both in-person and virtual engagement strategies.
• Deliver both individual and group counseling sessions using evidence-based therapeutic approaches tailored to meet the unique challenges faced by clients.
• Develop and implement personalized service plans that align with each client’s goals, fostering their individual growth and empowerment.
• Actively advocate for clients' rights and access to necessary resources, ensuring they have a strong voice in their care and social justice issues are addressed.
• Collaborate with various community organizations, government agencies, healthcare providers, and volunteers to enhance the support network available to clients.
• Participate in program evaluation, development, and grant reporting, contributing to the overall effectiveness and sustainability of our initiatives.
• Engage in training sessions, team meetings, and community outreach activities, fostering relationships that help inform and improve our services.

In this role, you will work directly with a diverse population, providing counseling services to individuals and families dealing with multifaceted issues, including grief, loss, mental health challenges, and social isolation. Your focus will be on empowering clients to navigate their circumstances and improve their quality of life, particularly in the sensitive setting of hospice care.

• Education: A Master’s degree in Social Work from an accredited institution is required.
• Licensure: Applicants must possess an active state license as a Licensed Clinical Social Worker (LCSW).
• Skills: Strong communication, empathetic interpersonal skills, and excellent organizational capabilities are necessary for success in this position.
• Knowledge: Awareness of social justice concepts, advocacy strategies, and trauma-informed care principles is essential.

• Prior experience working within a nonprofit organization or within a hospice or healthcare setting preferably related to social work.
• Familiarity with community resources and government services that benefit clients and enhance their well-being.

The annual salary for this position ranges from USD 75,000 to 100,000, commensurate with experience and qualifications, in addition to a generous sign-on bonus.
