Position Summary:
The Crisis Care Navigator is a member of the Crisis Services Team and serves individuals experiencing behavioral health crises across two primary service environments: the outpatient clinical office setting and hospital emergency departments. This position supports rapid engagement, care coordination, navigation, and continuity of care to ensure individuals receive timely intervention and connection to appropriate behavioral health services.
Key Responsibilities & Role Highlights:
  • Provide care navigation and support for behavioral health clients identified in the Emergency Department who do not require hospitalization.
  • Engage clients, gather required information, and coordinate follow-up care within 7 days of the ED visit.
  • Identify and address barriers to treatment engagement through outreach, advocacy, and resource coordination.
  • Arrange transportation and connect clients with community resources and behavioral health services.
  • Assist with insurance, funding applications, and service enrollment as needed.
  • Maintain accurate documentation and program data in accordance with agency requirements.
  • Collaborate with providers, community partners, and internal programs to ensure continuity of care.
  • Facilitate warm handoffs to ongoing treatment services and monitor engagement until transition is complete.
  • Participate in supervision, team meetings, trainings, and professional development activities.
  • Adhere to agency policies, procedures, and performance expectations.
Education & Experience Requirements:
  • Two or Four-year degree in social work, psychology, or related field preferred.
  • High School Diploma required.
  • Prior experience preferred in mental health, emergency services, case management, patient care, community resources, or other applicable healthcare setting preferred.
  • Must obtain CPI and CPR following hire.
  • Must be at least 21 years of age.
  • Must demonstrate knowledge of mental illness and have a general understanding of the treatment process.
  • Must be able to work a flexible schedule that may include evenings, weekends, and holidays.
  • Must demonstrate the ability to define problems, analyze data, establish facts and draw valid conclusions and make recommendations.
  • Must demonstrate a positive attitude and support for the mission of the agency.
  • Must demonstrate personal integrity and the ability to work as part of a team.
  • Must be able to work with ill, disabled, emotionally upset and at times emotionally dysregulated individuals.
  • Must be able to build relationships, collaborate and coordinate with external providers/partners.
  • Must be able to drive and be insured under the agency's commercial liability policy as a driver if hired in a program where driving is required.
