Behavioral Health Therapist

About Our Family:
Established in 1978, Alcona Health Center (AHC) is a non-profit, Federally Qualified Health Center dedicated to making a lasting impact in the Northern MI communities we serve, offering medical, behavioral health, dental and pharmacy services. As a member of the AHC family, you will join a team of over 300 coworkers, all devoted to providing quality health care to all residents of our communities, regardless of their ability to pay.

Your Valued Contributions:
  • As the Behavioral Health Therapist you will identify, triage and manage patients with behavioral health and substance use barriers within the primary care setting.
  • Assess patients’ with mental health, psychological, and/or substance use issues, create care plans, and manage care plans with a care team of primary care providers.
  • Conduct patient intakes, focused on diagnostic and functional evaluation, making appropriate mental health treatment recommendations to the primary care providers, and establishing positive rapport with patients. Provide individual, family, and group counseling. Provide episodic counseling services to patients in acute crisis.
  • Communicate with other care team members, including providers, regarding patients’ health as necessary. Advise primary care provider on which care settings best serve the needs of the patient. Follow-up with all patients and adjusts treatment plans as necessary. Refer patients to internal health center providers and resources as well as external providers and resources.
  • Assist patients with, and refer patients to, appropriate public benefits, vocational rehabilitation, social support, housing and other appropriate community resources. Provide patients with education regarding their care plans, any mental disorder self-management techniques, and any coping mechanisms for medication side-effects as necessary.
  • Utilize motivational treatment methods with patients with, or at risk for, physical disorders and help them make healthier lifestyle choices. Assist patients in complying with any medical treatment initiated by the primary care provider, such as offering strategies to cope with medication side effects.
  • Act as the health center resource for behavioral health. Provide training and assistance in the techniques/practices of tasks. Lead group discussions, problem solving and project activities as necessary. Provide behavioral health management consultation and training to clinic management, providers and other staff as appropriate.
  • Maintain all appropriate reports, charts, and documentation, including patient progress and diagnostic information.

Required Training amp; Experience:
  • Must possess a current CPR certification.
  • Must possess a current license to practice social work, counseling, or psychology in the state of Michigan (LMSW/LPC) and CAADC preferred.
