AlliedTravelCareers is working with Kiwi Healthcare to find a qualified Occupational Therapist (OT) in Tuba City, Arizona, 86045!

Our client is seeking an experienced Occupational Therapist for a Days shift position in Tuba City, Arizona. This Therapy role is based in the Occupational Therapist and requires a minimum of 2 years of recent Occupational Therapist experience.

Position Requirements

  • Active Arizona license and/or Compact license
  • Current BLS and/or ACLS and/or Specialty Certifications
  • Minimum 2 years of recent Therapy – Occupational Therapist experience
  • 2 current clinical references

Documentation Requirements

  • Current resume
  • Physical exam (within 12 months)
  • TB skin test (within 12 months)
  • Titers - MMR/Hep B/Varicella
  • Respiratory Fit Test (within 12 months)
  • Current Tdap and Flu vaccinations
  • Ability to pass background check and urine drug screening
