About the Role

The Care Coordination Peer Coach provides peer support, care coordination, and administrative assistance to members receiving behavioral health and medical services. This role helps ensure continuity of care by coordinating appointments, supporting member engagement, assisting with healthcare navigation, and serving as a liaison between members, providers, and internal teams. The Care Coordination Peer Coach promotes recovery, independence, wellness, and access to services while supporting a positive member experience.

  • Coordinating and tracking medical, behavioral health, telehealth, and related healthcare appointments
  • Assisting members with scheduling, appointment reminders, referrals, and healthcare navigation
  • Escorting members to onsite appointments and helping maintain continuity of care
  • Providing recoveryโ€‘oriented peer support and encouragement
  • Assisting members in identifying goals related to wellness, recovery, independence, and selfโ€‘advocacy
  • Supporting engagement in treatment, recovery programming, and community resources
  • Communicating member needs and barriers to care with clinical and operational teams

  • Must be 21 years of age or older
  • High school diploma or GED required (Associate's or Bachelor's degree in Behavioral Health, Psychology, Social Work, Healthcare Administration, Human Services, or a related field preferred)
  • Behavioral Health Technician (BHT) designation required
  • Peer and Recovery Support Specialist (PRSS) Certification required
  • Basic knowledge of patient care practices, medical terminology, healthcare documentation, and support services
  • Strong communication, organizational, criticalโ€‘thinking, and problemโ€‘solving skills
  • Proficiency with Microsoft Office applications and Electronic Health Record systems
  • Ability to work independently while collaborating effectively within a multidisciplinary team
  • Experience in substance use treatment, behavioral health, residential treatment, or integrated healthcare settings preferred
  • Experience using Kipu or other Electronic Health Record systems preferred
  • Additional peer support, recovery, care coordination, or case management experience preferred
