Physical Therapist (PT) – Home Health (Lymphedema)
Location: Treasure Coast, FL
Job Type: Full-Time / Part-Time / Per Diem
Setting: Field-Based Home Health
Reports To: Director of Clinical Services / Clinical Manager


Position Overview

We are seeking a compassionate and skilled Physical Therapist (PT) with lymphedema treatment experience to join our home health team serving patients throughout the Treasure Coast. This role focuses on delivering high-quality, individualized therapy services in patients’ homes while promoting functional independence, safety, and overall well-being. The ideal candidate is clinically confident, self-motivated, and comfortable working autonomously in a field-based environment.


Key Responsibilities
  • Evaluate and develop individualized treatment plans for home health patients.

  • Provide skilled physical therapy interventions in accordance with physician orders and care plans.

  • Perform lymphedema evaluations and treatment, including manual lymphatic drainage, compression therapy, bandaging, and patient education.

  • Monitor patient progress and modify treatment plans as needed.

  • Educate patients and caregivers on exercises, mobility techniques, fall prevention, and condition management.

  • Complete timely and accurate documentation in EMR system.

  • Communicate effectively with physicians, nurses, case managers, and interdisciplinary team members.

  • Ensure compliance with Medicare, state, and agency guidelines.

Qualifications
  • Current Florida Physical Therapist license in good standing.

  • Certified Lymphedema Therapist (CLT) or documented clinical experience treating lymphedema required.

  • Minimum 1 year of PT experience (home health preferred).

  • Current BLS/CPR certification.

  • Valid driver’s license and reliable transportation.

  • Strong clinical judgment and documentation skills.

  • Ability to work independently and manage a flexible field schedule.

  • Home health or geriatric experience.

  • Familiarity with OASIS documentation
