Intertwined Trauma-Informed Therapy Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team on a full-time basis. In this fully remote position, you will play a pivotal role in delivering trauma-informed care to patients at our affiliated hospice center in Jacksonville, Florida. • Conduct in-depth mental health assessments, ensuring detailed psychosocial evaluations, diagnostic interviews, and risk assessments are performed to understand each patient's needs thoroughly.
• Collaborate with patients to design personalized treatment plans, integrating individualized goals and cultural considerations to enhance engagement and effectiveness.
• Deliver evidence-based therapeutic interventions including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Solution-Focused Brief Therapy (SFBT) in various settings—individual, couple, family, and group therapy.
• Provide crisis intervention services, coordinating with additional healthcare providers to offer a network of support for patients facing acute psychological distress.
• Maintain up-to-date clinical documentation, including treatment plans and progress notes, in compliance with clinic policies and established regulatory standards.
• Empower patients and families through psychoeducation, facilitating knowledge transfer that equips them with the skills to effectively manage mental health challenges.
• Engage in regular collaboration with a multidisciplinary team of psychiatrists, psychologists, nurses, and case managers to deliver a holistic approach to patient care.
• Perform ongoing assessments of treatment outcomes to evaluate efficacy, adjusting treatment plans as necessary based on patient progress and feedback.
• Stay abreast of the latest research, best practices, and emerging trends in mental health to continuously enrich your clinical practice.
• Strictly adhere to ethical guidelines and protect the confidentiality and privacy of patient information at all times.

You will primarily serve a diverse patient population experiencing various mental health and psychosocial challenges, particularly those navigating the complexities of end-of-life care. Our clients often include individuals with a history of trauma, grief, anxiety, and depression, necessitating a tailored and empathetic approach to therapy that respects their unique circumstances and cultural backgrounds.

• A Master's degree in Social Work from an accredited institution is required.
• An active State license as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Proficient capability to work independently while also thriving in a collaborative, multidisciplinary team environment.
• Exceptional interpersonal and communication skills with a proven ability to establish rapport and trust with patients from diverse cultural backgrounds.

• Previous experience in a hospice or palliative care setting is highly desirable.
• Experience with trauma-informed care and evidence-based therapies.
• Familiarity with electronic health records (EHR) and other healthcare reporting tools.

The annual salary for this full-time position ranges from USD 70,000 to 84,000. Compensation will be commensurate with experience and qualifications.
