Position Overview
Inabia is looking for a Physical Therapist to join our team in a Rehabilitation setting. If you're passionate about patient-centered care and enjoy collaborating with a supportive team, we'd love to meet you. In this role, you'll evaluate patients, build individualized plans of care, and deliver skilled interventions that help people move better and live better. You'll partner closely with the interdisciplinary team and educate patients/caregivers to support safe progress and smooth transitions of care.Key Responsibilities
  • Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
  • Provide daily treatments, track functional progress, and support discharge planning.
  • Collaborate closely with therapy, nursing, and interdisciplinary teams.
  • Complete accurate, timely, and compliant clinical documentation.
  • Manage a steady outpatient caseload while maintaining quality of care and patient satisfaction.
  • Support clinic flow and communicate effectively with patients, families, and referring providers.
Qualifications
  • Active Wisconsin Physical Therapist license, or eligibility to practice in the state, required.
  • Strong communication, teamwork, and documentation skills.
Schedule
  • Shift: 5 x 8 Day shifts.
