At Transitions Hospice, we don't just manage care, we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means investing in the clinical excellence of every team member. From day one onboarding to ongoing professional growth, so that exceptional care is never the exception.

If you're an experienced hospice RN who is energized by teaching, mentoring, and raising the bar for clinical practice, this is your role.

What You'll Do

As Clinical Educator, you'll be a key member of the corporate clinical team; developing, implementing, and overseeing standardized education and competency programs across our multistate organization. You'll travel to branches, support new hires, close clinical practice gaps, and serve as a trusted resource for interdisciplinary teams from novice to expert. Your responsibilities include:

  • Developing teaching aids and educational materials that promote learning for both staff and patients/families
  • Evaluating the effectiveness of educational experiences to drive ongoing professional development and quality outcomes
  • Assessing educational needs and addressing clinical practice gaps in partnership with regional and local leadership
  • Aligning training initiatives with organizational goals, CMS regulations, Conditions of Participation, and hospice best practices

Leadership & Collaboration

  • Serving as a visible clinical resource and role model across multiple agencies and states
  • Collaborating with interdisciplinary teams, Administrators, and Executive Directors to develop agency goals and objectives
  • Demonstrating creativity and critical thinking in the development of patient care plans
  • Supporting audit readiness, regulatory compliance, and quality outcomes across all branches
  • Functioning independently, organizing assignments for maximum productivity across a multistate territory
  • Providing field support during staffing needs as determined by the Chief Nursing Officer
  • Travel required >70%

Benefits We Offer

  • Competitive pay
  • Medical / Dental / Vision Insurance plans
  • Long & Short-Term Disability
