About the Role:

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

Core Responsibilities:

  • Provide in-person medical care to patients in designated post-acute facilities
  • Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR
  • Collaborate with facility teams to support quality outcomes and smooth transitions of care
  • Collaborate as applicable in Trumen’s value-based care and transitional care programs
  • Develop strong working relationships with facility and clinical staff
  • Serve as a liaison between Trumen and facility teams
  • Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications:

  • MD or DO with a current, unrestricted Texas medical license
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
  • 1+ year of experience in post-acute, geriatric, or SNF care preferred
  • Comfortable practicing independently in skilled nursing facilities
  • Strong clinical judgment, communication skills, and a patient-centered mindset
  • Reliable transportation within the Madisonville/ Huntsville, TX area
