Description
Purpose
The HUB program is under the Ohio Commission of Minority Health, and woks to decrease infant mortality. LCCAA works with OCMH to administer HUB in Lorain County. Through Community Health Workers (CHW) who work directly with pregnant mothers (target is black pregnant mothers), in getting them the resources needed throughout their pregnancy (pre and post).
Summary of work activities, tasks and responsibilities
  • Plan, develop and implement strategies to utilize in meeting the Pathway Community HUB program goals and objectives in accordance with Ohio Minority Commission of Health requirements.
  • Develop and administrate policies, standards and procedures related to personnel, including staff development, budgeting, program performance; direct the activities of assigned staff, assign and review work.
  • Establish referral network of referral partners to the HUB to provide the aligned care coordination agencies with patients/clients to serve.
  • Develop and manage contracts with payers for the payment for Pathways outcomes and services for the HUB and its contracted care coordination agencies.
  • Market the Pathways HUB model of care coordination to the potential agency and referral partners with the intent to collaboratively integrate their services in the HUB.
  • Disseminate community information gathered by the Pathways HUB and its aligned agencies to broaden the positive awareness and acceptance of the benefits of a Pathways Community HUB.
  • Establish and maintain effective community relationships with other community based and governmental agencies and organizations to facilitate the meeting of community needs and services.
  • Direct and coordinate public relations, fundraising, and fact finding or research activities.
  • Develop, distribute and maintain program and program related reports.
  • Attend conferences, meetings workshops and seminars as required.
  • Abide by applicable agency rules and regulations.
  • Abide by agency policies and procedures.
  • Work with executive team members.

Education required:
Bachelor degree required.

Physical requirements:
  • Ability to hear and understand speech at normal levels and on the telephone with or without hearing aids.
  • Ability to lift and carry up to 20 lbs. at waist height within an office setting.
  • Ability to see and read a computer screen and printed matter with or without vision aids.
  • Ability to see, hear and speak with/without assistive devices sufficient to communicate effectively with others.
  • Ability to sit at a desk or workstation for extended periods of time.
