Key Qualifications – Registered Nurse (RN)

  • Provide subsequent skilled nursing visits and ongoing home health nursing care in patients' homes

  • Document all care accurately and promptly using electronic medical records (EMR/HCHB)
  • Collaborate with interdisciplinary teams, case managers, and physicians to ensure optimal patient outcomes
  • Participate in on-call and weekend rotations as required
