Job description

Join a mission-driven, community-focused health system serving Brooklyn. We seek a compassionate Primary Care Physician (Internal Medicine or Family Medicine) to deliver comprehensive, patient-centered care in a collaborative, multidisciplinary setting that emphasizes clinical excellence, innovation, and community impact.

Responsibilities

  • Provide outpatient primary care and longitudinal management for a diverse patient panel.
  • Manage population health initiatives and participate in value-based care programs.
  • Coordinate care with multidisciplinary teams, specialists, and community resources.
  • Participate in teaching and supervision of trainees and support graduate medical education.
  • Document care in the electronic health record and meet quality, productivity, and reporting expectations.

Compensation & Benefits

  • Total compensation $180,000–$320,000 annually (base + productivity incentives, based on experience/training).

Requirements

  • MD or DO from an accredited medical school.
  • Board Certified or Board Eligible in Internal Medicine or Family Medicine.
  • Completion of an accredited Internal Medicine or Family Medicine residency.
  • Eligible for or able to obtain an unrestricted New York medical license prior to employment.
  • Current BLS certification.
  • Experience with Epic or a comparable enterprise EHR.
  • Cultural competency and/or multilingual skills relevant to eastern Brooklyn populations (Spanish, Haitian Creole, Bengali, or other commonly spoken languages) preferred.
  • Interest in value-based care, population health, care coordination, and teaching.
