Job Title:Physical Therapist (PT)
Job Location: Harford, PA and Montrose, PA. Our Physical Therapists see patients in the most ideal setting - the patient's home!
Our office are located in Dunmore, PA. At Traditional, we do our best to keep each PT's case load within a sensible geographic territory.
Shift(s): Full-time/Part-time positions available. Monday through Friday 8:30am to 5:00pm
Duties and Responsibilities include, but not limited to the following:
  • Evaluate patients and develop plans of care to achieve desired outcomes
  • Provide therapeutic treatment modalities and implement home therapy / exercise programs
  • Provide instruction to patients to achieve patient goals
  • Maintain communication with ordering physicians and clinical team to promote progress toward goals or seek intervention when needed
  • Participate in quality improvement activities as assigned
  • Timely and accurately document (EMR) all patient assessments, interactions, etc.

Requirements:
  • Must possess a current, valid Pennsylvania PT license and have current CPR certification
  • Valid driver's license and ability to travel to case assignments
  • Must be comfortable working in home setting
  • Ability to work independently with minimal supervision and manage time effectively
  • Strong interpersonal and communication skills
