$500 CONTRACT COMPLETION BONUS
Opportunity:
RN - Home Health Nurse
Job Details:
• Monday through Friday schedule from 8:00 AM to 4:30 PM (end time may vary)
• Weekend requirements limited to on-call shifts only
• On-call requirement is 2 times per week, Monday through Thursday
• Will see 5-6 patients per day depending on type of visit, 25-30 points per week
• Visits will include routine and OASIS visits
• Will perform both home health and hospice visits - no floating between units
• Common patient diagnoses include Heart Disease, CHF, COPD, Pneumonia, Dementia/Alzheimer's, Depression, Stroke, Cancer, Kidney Disease, Diabetes, Liver Disease, Wounds, Joint Replacement, and Diabetes
• Primarily geriatric patient population with occasional pediatrics
• Required skills include Critical Thinking, Chronic Disease Management, Patient/Family Education, NPWT, Ostomy Care, Central Line Care, IV Administration, and Complex Wound Care
• Hospice experience required: pain and symptom management, end-of-life care, empathy, and compassion
• Weekly IDT meeting and case conference at office; monthly all-staff meeting required
• No vents required; CPAP and BiPap used on occasion with patients self-managing
• Charting system: Cerner
• Dress code: Solid color scrubs (no patterns or logos unless company-issued); closed-toe, non-slip shoes with socks or hosiery required; no flip-flops, open-toe shoes, or sandals
• Required certifications: BLS (AHA)
• BSN not required
• Travel experience preferred

This post-acute care facility operates a dedicated Home Health and Hospice department, providing compassionate in-home care to patients across the community. The department is supported by a team that includes certified nursing assistants and administrative leadership available after hours, on weekends, and during holidays. The facility utilizes Cerner as its charting system and provides structured orientation including computer-based training and field shadowing to ensure clinicians are well-prepared for their roles.
