Job Summary:

Essential Duties and Responsibilities
  • Support and promote the mission and values of Covenant Health Ministry.
  • Monitor and manage operations of patient throughput, overseeing admissions, discharges, and transfers to ensure efficient bed utilization and efficient care delivery through optimal resource utilization.
  • Strong collaboration with Hospitalist Medicine and Care Coordination.
  • Direct oversight of the Discharge Lounge and Multidisciplinary Rounds.
  • Work collaboratively with the nursing supervisor and nursing leadership to monitor bed availability and to optimize patient placement to enhance overall hospital efficiency.
  • Facilitate code purple events and capacity huddles to optimize patient throughput.
  • Work collaboratively with transport team and admission, discharge RNs to optimize patient throughput and discharges.
  • Maintain responsibility for bed capacity/management utilizing bed placement and patient throughput tools.
  • Act as administrative liaison and maintain relationships with external transport agencies, collaborating with St Joseph Hospital EMS manager,
  • Utilize evidence and research to develop a strategic plan to develop and implement policy, practices, and workflows that optimize patient throughput.
  • Mitigate barriers to discharge by rounding in departments and attending MDRs to provide assistance and support to optimize patient throughput with authority to request plan adjustments in the best interest of the patient.
  • Maintains budgets and resources related to patient flow operations, provides monthly updates to CFO and CNO.
  • Attend Hospitalist staff meetings and Patient Care Services Executive meetings.
  • Develop and maintain relationships with external partners to facilitate seamless patient transitions.
  • Maintain responsibility for patient throughput metrics and data analysis to identify areas for improvement and implement Evidence-Based solutions.
  • Coordinates patient flow data to identify trends, forecast bed availability and prepare reports allowing for leadership to make informed decisions regarding resource allocation.
  • Lead patient throughput initiatives and maintain accountability to enhance operational efficiency and patient experience, including the development of key performance indicators (KPIs).
  • Present data analysis and KPI progress monthly at patient throughput meetings, provide input into monthly agenda.
  • Act as resource and mentor for healthcare team members regarding patient throughput, fostering a culture of continuous improvement and innovation.
  • Collaborate with various departments and stakeholders to identify and address bottlenecks and inefficiencies in patient flow.
  • Collaborate with medical providers, acting as a resources and partner to positively impact patient throughput.
  • Stay up-to-date on industry trends and best practices related to patient throughput and develop recommendations for improvement.
  • Ensure compliance with relevant regulations and policies related to patient throughput.
  • Communicate regularly with senior leadership to provide updates and recommendations on patient throughput initiatives.
  • Foster a culture of continuous improvement and innovation within the hospital, act as champion and resource for change.
  • Maintain a high level of customer service and patient satisfaction throughout the patient flow process.
  • Maintain responsibility for training healthcare team members on patient throughput policy and procedures to ensure that all team members are equipped to manage patient throughput effectively.
  • Effectively communicate and engage with internal and external stakeholders to assess satisfaction and improve patient throughput.
  • Maintain nursing competencies, as appropriate, to meet or exceed PSC requirements and to support the patient care team.
  • Other duties as consistent with this role.

Education and Experience
  • Registered Nurse Graduate of an approved clinical Nursing Program
  • Current NH RN License required
  • RN preferred with minimum seven years of clinical experience, experience in supervision or bed placement/patient throughput preferred.
  • BSN required
  • MSN preferred
  • Knowledge of EPIC Electronic Health Record preferred
  • Basic Cardiac Life Support required
  • Certification in clinical specialty or nursing leadership preferred.
