About Us

Concept Therapy PC and Concept Therapy Home Health provide high‑quality therapy services across outpatient clinics, assisted living communities, and in‑home skilled care settings. We focus on patient‑centered treatment, strong interdisciplinary collaboration, and a supportive work culture that values growth, autonomy, and clinical excellence.
We are expanding and looking for a compassionate, motivated Occupational Therapist to join our team. This role includes opportunities in both outpatient therapy and skilled home health, allowing you to practice across settings and support patients throughout their full continuum of care.
Job Summary

As an Occupational Therapist with Concept Therapy, you will work with patients from diverse backgrounds, evaluate their functional needs, and develop customized therapy plans that help them reach meaningful goals. You will collaborate closely with physicians, nurses, therapists, and caregivers to ensure coordinated follow‑up care and provide education that empowers patients to thrive.
The ideal candidate is compassionate, organized, communicative, and comfortable managing a varied caseload across outpatient and home health environments.
Responsibilities

  • Meet with patients from a wide range of backgrounds and discuss their therapy needs with empathy and professionalism
  • Conduct thorough evaluations to establish functional baselines

  • Provide outpatient treatment in clinic settings and skilled therapy services in the home

Qualifications
