Job Description:

The Physical Therapist will provide services within a home health setting covering four counties, primarily rural with mid-sized towns. The role requires completing Oasis Assessments, Start of Care cases, and managing patient visits using a tablet track system. Typical cases include elderly falls and fractures. Business casual or Hunter green scrubs with a white or black top are required.

Responsibilities:

  • Complete Oasis Assessments and Start of Care cases
  • Conduct 4-6 patient visits daily using a point system

Requirements:

  • Must have home health experience as well as Home Care Home Base experience
  • MUST HAVE LICENSE IN HAND UPON SUBMITTING
  • BLS (AHA) certification required
