Jackson Therapy Partners is seeking a travel Home Health Physical Therapy Assistant for a travel job in Clackamas, Oregon.

Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active Oregon PTA license required to start the assignment.
  • Home health experience in Oregon required.
  • BLS/CPR certification required (AHA or ARC accepted).
  • Experience with patient subsequent visits and home health documentation required.
  • Valid driver’s license and ability to travel between patient homes within Multnomah and Clackamas counties. Mileage reimbursement provided between visits.
  • Preferred: Experience with HCHB (Homecare HomeBase) electronic documentation.


Assignment Details

  • Facility type: Home Health.
  • Shift: Evenings.
  • Shift hours: 2000-0400.
  • Weekly hours: 36 guaranteed per client notes.
  • Coverage area: Multnomah and Clackamas counties; travel between patient homes required.
