Job Summary:

Polaris Healthcare and Rehabilitation Center is seeking a caring and compassionate Physical Therapist to join our rehab team! As a Physical Therapist with our team, you will have the opportunity to make a difference in the lives of our residents by helping them achieve their optimal physical performance. Our mission is to meet the physical, emotional, and spiritual needs of our residents in a compassionate environment that promotes dignity and respect.

Responsibilities:

• Develop individualized plans of care to facilitate optimal performance using standardized tests and measures as evidence for improvement
• Identify needs and make referrals to other disciplines to address the comprehensive needs of patients
• Monitor patients' progress and adjust treatments accordingly
• Provide supervision for Physical Therapy Assistants, students, and support personnel in accordance with state license requirements and professional standards
• Perform other duties as assigned to provide quality care to residents

Qualifications:

• Experience providing physical therapy to patients in a skilled nursing facility
• Graduated from an accredited physical therapy program
• Current Physical Therapy license for the state of Delaware
• Current CPR certificate
• Good organizational, time management, and communication skills
